Please fill out the details below accurately.  You will be required to select what category you are entering the team into, please select as appropriate.

A minimum of 7 players per team are required. 

Only 8 teams per category will be accepted. 

You will receive an acknowledgement email immediately after completing the form.  Please ensure that all the details have been successfully submitted.
